Schoolcraft COVID-19 screening form

Effective immediately, we will be using the Schoolcraft COVID-19 screening form. This will be a requirement for every session / game for athletes and upon entrance inside for guests.

Check In

Players will be “Checked In” when indoors by the facility manager using the email or text confirmation.

Guests

No guests are allowed for training (except players U8 and below).

2 guests are allowed for games inside, starting November 7, as long as the screening form is completed along with a Check in.

Masks

As aligned with MSYSA requirements, masks required at all times for players outside their vehicles and when indoors.

Social Distancing

Efforts to maximize social distancing are expected at all times including during out-state travel.
